Synergy Hospice Care is a licensed and accredited hospice care services provider in the State of California and Medicare-certified to provide care in Los Angeles, Orange, Riverside and San Bernardino counties.

CO-OWNER / CEO / ADMINISTRATOR

MINIVER CHOI

Miniver is a compassionate healthcare leader with a unique blend of experience and dedication that guides Synergy Hospice Care.

As Co-Owner, Administrator, and CEO, Miniver is committed to providing high-quality, patient-centered hospice care through a team of skilled professionals who share her passion for empathetic service. Every decision she makes is driven by a desire to support patients and their families during life’s most challenging times.

Her role as a Board Member of Little Tokyo Towers Corporation, which serves low-income seniors in Los Angeles, reflects her lifelong dedication to community well-being and social equity. This background in nonprofit leadership, combined with her extensive experience in private equity and investment management, uniquely equips Miniver to lead Synergy Hospice Care with a focus on ethical practices, transparency, and a commitment to high standards in care delivery.

Miniver’s approach emphasizes both compassion and integrity, providing families with a trustworthy, supportive partner in their care journey.

CO-OWNER / CFO

BRENT CHAPPELL

Brent brings over 30 years of real estate investment experience, with a specialized focus on senior healthcare, to his role as Co-Owner and CFO of Synergy Hospice Care.

His leadership roles in healthcare REITs, such as Sabra Health Care, LTC Properties, and Ventas, have given him a profound understanding of compliance, ethical governance, and financial transparency, which surpasses that of many hospice operators. His expertise ensures that Synergy Hospice upholds the highest standards of regulatory adherence, instilling trust among patients and families alike.

Brent also holds Administrator certifications in Residential Care for the Elderly (RCFE) and Adult Residential Facility (ARF), deepening his insights into quality care standards and compliance. With a Master’s in Aging Services Management from USC and an MBA from UC Irvine, Brent brings both financial expertise and compassionate values to Synergy Hospice.

Brent's dedication to balancing patient-centered care with ethical operations sets a high standard for the industry, positioning Synergy Hospice as a trustworthy, integrity-driven care provider.

DIRECTOR OF PATIENT CARE SERVICES

JACQUELINE BAEZA, FNP, MSN, PHN

Jacqueline exemplifies a steadfast commitment to compassionate care as the Director of Patient Care Services at Synergy Hospice Care.

With extensive experience in skilled nursing and hospice care, Jacqueline provides holistic, individualized support that respects each patient’s physical and emotional needs.

Her background as a registered nurse includes multiple leadership roles where she has consistently maintained high clinical standards, overseen budget management, and nurtured a compassionate environment for both staff and patients. Jacqueline’s pursuit of advanced education as a Family Nurse Practitioner (MSN) demonstrates her commitment to professional growth, allowing her to offer a high level of clinical expertise in hospice care.

Jacqueline's compassionate approach and deep regulatory knowledge ensure that each patient at Synergy Hospice receives care that is empathetic, respectful, and ethically grounded.

GABRIELA CUEVAS

Gabriela brings extensive knowledge of hospice operations to her role as Office Manager at Synergy Hospice Care.

With a strong foundation in facilitating patient services and coordinating essential administrative processes, Gabriela ensures that Synergy’s operations run smoothly and efficiently for both patients and their families.

Her expertise in patient referrals, intake management, and insurance verification, combined with her commitment to compassionate communication, provides families with clarity and support throughout the care process.

Gabriela’s bilingual abilities allow her to connect with diverse communities, ensuring that all patients feel welcome and supported. Her experience in hospice and healthcare administration positions her to understand and address the unique needs of hospice patients.

COMMUNITY LIAISON

JACQUELINE KREGLER, MSW

Jackie’s heartfelt dedication to compassionate, patient-centered support defines her role as Community Liaison at Synergy Hospice Care.

With a Master’s in Social Work and a background in clinical psychology, Jackie brings an exceptional ability to craft personalized case management plans for each patient. Her diverse experience—from mental health support to youth counseling, family therapy, and hospice care—enables her to provide comprehensive, resource-based support that addresses every aspect of a patient’s well-being.

Jackie's experience with vulnerable populations, including hospice patients and those in the justice system, gives her a deep understanding of the resources available to enhance patient care. Her certifications in Motivational Interviewing and social work bring an added level of empathy and advocacy to her role.
