Synergy Hospice Care is a licensed and accredited hospice care services provider in the State of California and Medicare-certified to provide care in Los Angeles, Orange, Riverside and San Bernardino counties.

Meet the Team
Synergy Hospice Care is led by an interdisciplinary team with experience in hospice medicine, nursing leadership, behavioral health, social work, healthcare operations, and care coordination. Together, our team supports patients and families facing complex medical, behavioral, and social challenges and works closely with hospitals, physicians, and community providers to help navigate difficult care transitions and placement situations.
MINIVER CHOI
CEO & ADMINISTRATOR
Ms. Choi oversees agency operations, regulatory compliance, strategic growth, and referral partnerships for Synergy.
She brings experience in licensed home care operations, senior housing, hospice administration, and healthcare-related investments. Her background includes healthcare and senior housing investment, underwriting, portfolio management, and operator relations within the senior housing and post-acute care sectors.
Ms. Choi works closely with hospitals, physicians, case managers, and community partners to help address complex care coordination and placement challenges. She collaborates with referral sources to help identify safe and appropriate care solutions for patients whose medical, behavioral, or social circumstances may complicate traditional hospice placement.
Ms. Choi co-founded Synergy in 2023.

ASHWANI BHARDWAJ, MD
MEDICAL DIRECTOR
Dr. Bhardwaj provides medical leadership, supports hospice eligibility determinations, and collaborates closely with the interdisciplinary team to ensure care delivery aligns with CMS Conditions of Participation and California regulatory requirements.
Dr. Bhardwaj is board certified in Internal Medicine and Hospice & Palliative Medicine and brings more than two decades of clinical experience caring for patients with complex, chronic, and life-limiting illnesses. Dr. Bhardwaj has practiced in California since 1998 across inpatient and outpatient settings.
Dr. Bhardwaj joined Synergy in November 2024 as Hospice Physician and assumed the role of Medical Director in April 2025.

ELAINE SIMON, BSN, RN
DIRECTOR OF PATIENT CARE SERVICES
Ms. Simon leads clinical operations and oversees the delivery of hospice services, supporting safe, high-quality care for patients with complex medical and social needs.
Ms. Simon brings more than a decade of hospice nursing and leadership experience, including oversight of interdisciplinary teams, clinical quality, and regulatory compliance. She has held Director of Patient Care Services roles at multiple California hospice agencies.
Ms. Simon joined Synergy in January 2025 and previously served as Clinical Supervisor and DPCS Designee prior to her formal appointment as DPCS in January 2026.

PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER
Mr. Le supports patients with psychiatric, behavioral, and psychosocial needs as part of Synergy's interdisciplinary team. His involvement strengthens the organization's ability to care for patients whose medical conditions are complicated by co-occurring mental health or behavioral challenges.
Mr. Le is board certified as a Psychiatric Mental Health Nurse Practitioner and brings experience in inpatient and outpatient psychiatric care, psychiatric assessment, medication management and crisis intervention.
Mr. Le joined Synergy in June 2025.

Ms. Alvarez supports patients and families through psychosocial assessments, counseling, crisis intervention, resource coordination, and care planning as part of Synergy's interdisciplinary team.
Ms. Alvarez brings experience across hospice care, acute care, emergency services, HIV programs, behavioral health, and county-based social services. This background enables her to assist patients and families facing complex medical, social, and environmental challenges, including limited caregiver support, housing instability, and behavioral health concerns.
Ms. Alvarez joined Synergy in October 2025.

Mr. Chappell is responsible for financial oversight, strategic planning, and organizational sustainability at Synergy.
Mr. Chappell brings more than 35 years of experience in commercial real estate investment and management, including nearly two decades focused on senior housing and healthcare-related real estate. His background includes executive leadership roles with publicly traded healthcare real estate investment trusts, overseeing healthcare portfolios, operations relationships, and strategic initiatives across the senior housing and post-acute care sectors.
Mr. Chappell co-founded Synergy in 2023.
